How do I find an In-Network doctor?
To find an in-network doctor, follow the instructions in the "How to Find a Provider" video under the "Videos" section of this website, and use the provider look-up link on the website.
When does coverage start?
Plans begin on the 1st of the next month, as long as you've completed your enrollment by the 25th of the prior month.
When is the deadline to apply/enroll?
You can enroll at any time throughout the year. If you enroll by the 25th of the current month, you can choose to have your plan start on the 1st of next month!
Do need to fill out a health questionnaire?
No, the online enrollment form is all you will need. Be ready with things like, Dates of Birth, Social Security Numbers, and bank account information or a credit card.
How long is the application?
Not long! Most enrollments can be completed in 10 minutes.
Are pre-existing conditions covered?
Yes, on the Atlas plans, in accordance with the plan brochure. ShareWise does have pre-existing condition limitations, please review the plan brochure for full details.
When will my monthly premium draft occur?
If you're using a credit or debit card for your monthly premiums, the charge will occur on the 25th of the month. If you are using a bank account, the ACH draft will occur on or around the 27th of the month. Payment occurs in the month prior to the month of coverage.
Do I need to renew once a year?
No. Your coverage will auto-renew until you cancel.
Will I get cards in the mail?
If you're on an Atlas plan, you will get cards in the mail. Look for a plain white envelope from "Tall Tree Health Administrators" or "National Combined Benefits Associations". If you are on the ShareWise Plan, you will log into your member portal to print ID cards. The Welcome Letter that you receive upon enrollment will contain access to the member portal.
I am having trouble logging into the member portal. What do I do?
Existing members can contact Member Services by email at [email protected] or calling the number on your card and pressing the option for customer service.

I’m on Medicare. Will one of your plans work for me?
At this time, we do not have any plans compatible for individuals over the age of 65.
